2. The department head pre-approves the request. You will be required to substitute any accrued paid leave while on unpaid personal leave.

When the employee returns from personal leave, he or she will resume the same job or a comparable job. The employee and his or her dependents will be covered by company-paid benefits during the leave. The employee must make arrangements to pay his or her payroll contribution rates by personal check while on personal leave. If employment with another company occurs during the leave, the Company will consider the employee to have voluntarily quit or resigned and all reinstatement rights are immediately forfeited. MILITARY LEAVE There is no employment service requirement that must be satisfied for military leave. If you are inducted into the U. S. Armed Forces, or if you have reserve duty training obligations, you will be given a leave of absence. We request that you provide us with as much advanced notice as possible. Upon completion of your duties, you will have reinstatement rights consistent with applicable laws and business requirements. Additionally, as a company we have a military leave supplement based upon the difference between your military base earnings and your base company earnings. This is payable for six months, if you qualify. Please see your human resources business partner for additional information. If you comply with all applicable legal requirements concerning reinstatement, there will be no waiting period required for reinstatement of company benefits upon your return. The human resources business partner can provide counsel on requirements and rights as requested. BEREAVEMENT LEAVE Bereavement Leave for an Immediate Family Member When a death occurs in an employee's immediate family, including the death of an employee’s unborn child, all regular full-time employees may take up to three (3) days off with pay to attend the funeral, make funeral arrangements or to grieve, if the employee is unable to attend the service due to time or travel constraints. This benefit is separate from the Company’s PTO or vacation policies. Bereavement leave will be paid at the regular straight time hourly rate up to a maximum of eight (8) hours per day. Paid bereavement leave does not count as time worked for purposes of calculating overtime. In recognition of the diversity among our employees, bereavement leave must be taken within 180 days of the event. Bereavement time may be taken intermittently in full-day increments with your manager’s approval, unless state or local laws provide otherwise. Bereavement leave provided under this policy runs concurrently with Immediate family members are defined as an employee's spouse, parent, children, siblings, grandparents, or grandchildren. All immediate family relationships recognize and include relatives that may be half, step, or current in-laws. Additional Paid Time Off The Company understands the deep impact that death can have on an individual or a family, therefore additional paid time off may be needed. The employee may make arrangements with his or her manager to take additional time through his or her available vacation or PTO bank. Additional vacation/PTO may also be granted depending on circumstances such as distance, the individual's responsibility for the funeral arrangements, and the employee's responsibility for taking care of the estate of the deceased. protected funeral or bereavement leave under state or local laws, if applicable.
