ACKNOWLEDGMENT OF DRUG AND ALCOHOL POLICY I have received a copy of the Drug and Alcohol Abuse Policy of Flowers Baking Co. of Jacksonville (The Company). I have read and understand this policy. Specifically, I understand that compliance with the drug and alcohol policy is a condition of employment and violations will be subject to discipline up to and including termination of employment. It is my responsibility to ensure that I understand all provisions of this policy and if I have any questions, I should ask my manager or HR Department. I understand that I am subject to and voluntarily consent to random, reasonable suspicion, and post accident drug and/or alcohol testing. I further understand that if I engage in any of the following, such conduct may result in disciplinary action, including, but not limited to, the termination of my employment with Company: 1) my refusal to submit to a random, reasonable suspicion and/or post-accident drug and/or alcohol testing; 2) my refusal to permit the testing medical provider and/or laboratory from sharing the results of such testing with Company; 3) providing false information in connection with the testing to Company and/or the testing medical provider and/or laboratory; or 4) engaging in any conduct that tampers with the test results. I further understand that the Company has an Awareness Program that can provide assistance if I believe I have a substance abuse problem; however, it is my responsibility to inform the Company prior to being caught in violation of the drug and alcohol policy or being requested to take a random or reasonable suspicion drug/alcohol test.
