a. Late reporting of an accident, vehicle theft, or rental-car incident. These incidents should be reported within 2 hours of the event but must be reported by the end of the workday during which the incident occurred. Reporting is considered late if it occurs after the end of the workday in which the accident, theft, or rental-car incident happened, except in certain acceptable mitigating circumstances. b. Late reporting of vandalism to fleet vehicles. c. Preventable rear-end accidents where the CMV driver hit another vehicle. d. Failure to submit an original DOT inspection form and copies of the repair receipts within 24 hours of any DOT inspection resulting in safety defects.
