increments up to a guaranteed issue amount $50,000. Your election includes an equal amount of AD&D coverage.

SHORT-TERM DISABILITY The company offers all employees, at no cost to the employee, Short-Term Disability (STD) for a period not to exceed 12-weeks. The company’s STD plans are a payroll practice and benefits are not paid by an insurance company. Please refer to the terms of the STD plan applicable to your position for more information regarding your STD benefits. A copy of the applicable STD plan may be obtained from the Human Resources Department. VOLUNTARY BENEFITS The company offers several additional voluntary benefit plans that can help you offset expenses associated with health care related to illness or injury and/or provide other valuable benefits. You pay the full cost of these premiums via payroll deductions. For additional details on these plans, visit FLObenefits or see your local HR team. COBRA AND CONVERSION OF BENEFITS If you or your covered dependents experience a qualifying loss of coverage, you may be eligible to continue certain benefits through COBRA or other conversion of benefits. These options will be communicated to you upon the loss of coverage. PAID TIME OFF (PTO) Everyone needs to have time off for rest and relaxation. The company has a Paid Time Off (PTO) policy. Each week of your employment, you earn a certain amount of PTO based on your length of service. You earn PTO from the first day of your employment until the last active day of your employment. An account is established for you in which the company credits the appropriate fraction of your annual PTO amount each week (e.g., 12 days = 96 hours per year = .1.846154 hours per week). Please refer to your specific Paid Time Off or your specific Paid Time Off and Attendance Policy for individual details. Employees who work a reduced work schedule may still participate in the company’s PTO program if the employee works at least 30-hours per week. The accrual of PTO time is based upon the employee’s reduced pay and/or number of hours worked. The company strongly encourages you to use your PTO during the year in which it is earned. Employees will stop earning PTO whenever their PTO account reaches their annual accrual amount. Refer to your specific PTO and Attendance policy to determine your annual accrual limit. The accrual will resume after PTO is used and deducted from the account. No payment will be made in lieu of time off. Accrued PTO is not payable at termination until an individual has completed the 30-day benefits eligibility waiting period . PAID HOLIDAYS All eligible non-exempt employees who work their scheduled day immediately before and following the holiday (and the holiday if scheduled to work), shall receive an amount equal to 8 hours straight-time pay for the holiday.
