1.3.6.b.6 An employee’s legible, written statement concerning status of the employee or the employee’s family member as a victim of domestic violence, sexual violence, abuse, or stalking that signals the employee’s identity and (if applicable) relationship to the family member

NOTE: The PST Act is designed to allow you paid, job protected time away from work to see to issues that will arise from life. Proper usage of PST helps protect you financially when the need arises and it also helps protect your job. It is NOT meant as an extra week of vacation. You are entitled to the full 40 hours per accrual year but you are also cautioned to use your paid sick time wisely. See our policy on Paid Sick Time for additional information. 1.4 MISCELLANEOUS 1.4.1 Doctor’s Certificate : A doctor’s certificate may be required by the Company to establish “fitness for duty.” Additionally, if you are taking PST for your health care or for the health care needs of a family member and are out 3 or more consecutive days, you will be required to present appropriate documentation from a health care provider so that the company can fulfill its obligations under both the Family and Medical Leave Act and The PST Act. 1.4.2 Proper Notice : Proper notice is defined as a minimum of two (2) hours’ notice before the scheduled start of your shift. Such notice must be delivered to the designated Supervisor to be considered proper. 1.4.3 Pre-approved Excused Absences (unpaid): Employees may request approval for up to six (6) pre-approved excused absences per calendar year. An employee requesting time off under this Section must complete and submit a Time Off Request form to his or her supervisor at least two (2) weeks in advance. The supervisor will send a copy of the request to Human Resources. a. Interaction of Pre-approved Excused Absences and PST: This type of pre-approved excused absence may meet the definition of absences eligible for PST. If the absence meets PST requirements – and you have PST still available – PST will apply and you will be paid for the absence up to 8 hours providing you have that much paid sick time available. The absence will not count toward the six (6) excused absences. If the absence meets the PST requirements and you do NOT have PST available for the accrual year, the pre-approved excused absences (unpaid) provisions will apply and the absence will count toward one of your six (6) excused absences per calendar year. 1.4.4 Excessive excused absence or tardiness. Excessive excused absences or tardiness may result in disciplinary action, depending upon the circumstances of each case. The Company defines excessive excused absences as any pattern of absences that become a disruption to the workforce. The company defines excessive tardiness as any pattern of tardiness that becomes a disruption to the workforce. Where appropriate, employees with an excessive number of excused absences may be required to take a leave of absence. This rule does not apply to any absences or tardiness covered by legally protected leave such as FMLA or The PST Act.
