BONE MARROW DONATION LEAVE
Employees may be eligible to take up to five (5) days’ paid leave to undergo a medical procedure to donate bone marrow in any one-year period.
To the extent possible, employees must provide reasonable advance written notice of their need for leave under this policy including verification by a physician of the purpose and length of each requested leave.
When possible, employees should consult with their supervisor to schedule the leave so that it does not unduly disrupt Company operations.
During the period of time the employee is granted such leave, the Company will maintain the same level of coverage of benefits the employee had prior to taking leave.
Upon expiration of the leave, an employee will generally be reinstated to his or her position with equivalent seniority, benefits, pay and other terms and conditions of employment.
Employees must use up to five (5) days accrued and unused vacation time or paid sick leave for this purpose.
ORGAN DONATION LEAVE Employees may be eligible to take up to 30 days’ paid leave for organ donation in any one-year period. Employees may also be eligible for an additional unpaid leave of absence of up to 30 days in a one-year period. To the extent possible, employees must provide reasonable advance written notice of their need for leave under this policy including verification by a physician of the purpose and length of each requested leave. When possible, employees should consult with their supervisor to schedule the leave so that it does not unduly disrupt Company operations. During the period of time the employee is granted such leave, The Company will maintain the same level of coverage of benefits the employee had prior to taking leave. Upon expiration of the leave, an employee will generally be reinstated to his or her position with equivalent seniority, benefits, pay and other terms and conditions of employment. Employees must use up to two (2) weeks accrued but unused vacation time for this purpose. MILITARY SERVICEMEMBER CARE LEAVE Employees who are eligible for leaves of absence under the Company’s CFRA/FMLA policy are entitled to military leave under federal law for either of two reasons. An eligible employee may request up to 12 weeks’ leave because of “any qualifying exigency” arising out of the fact that the spouse, son, daughter, or parent of the employee is on active duty, or has been notified of an impending call to active duty status, in support of a contingency operation. An eligible employee who is the spouse, son, daughter, parent, or next of kin of a covered servicemember who is recovering from a serious illness or injury sustained in the line of duty on active duty is entitled to up to 26 weeks’ leave in a single 12-month period to care for the servicemember. This military caregiver leave is available during a “single 12-month period” during which an eligible employee is entitled to a combined total of 26 weeks of all types of family and medical leave.
