ATTENDANCE POLICY The orderly and efficient business operation of Mesa Organic Baking Co., Inc. (the “Company”) requires that all employees be at their assigned work station, ready and able to work, at their assigned starting time on each and every scheduled work day. Accordingly, it is the purpose of this company policy to define acceptable attendance and set forth the administrative guidelines for dealing with attendance issues, including excessive absences. Absences covered by any legally protected leave, such as the Family and Medical Leave Act or Arizona’s Fair Wages and Healthy Families Act (The PST Act) are not counted or otherwise considered for disciplinary or any other adverse action. 1.1 PROCEDURE Employees accumulating points will receive the progressive disciplinary action noted below based on the number of points accumulated during any rolling 6-month period. Active Points Total Disciplinary Action Taken 0 Excellent Record 1 Documented Verbal Warning 2 1 st Written Warning 3 2 nd Written Warning 4 1 day of Suspension without pay (This does not apply to salaried exempt employees) 5 Termination of Employment Attendance policy points will remain active for six (6) months from date of occurrence. For purposes of this policy, “Suspension” is defined as one (1) complete shift. A second attendance related suspension in a rolling 12 month period will subject the employee to termination without additional warning 1.2 DESCRIPTION OF OCCURRENCES 1.2.1 Tardy : One (1) point will be charged if later over 4 hours. Half (1/2) point will be charged if late within 4 hours. A tardy will begin five (5) minutes after your scheduled shift starts. Any employee reporting for work (clocking in or signing in) more than one hour after his or her scheduled starting time, may at the option of the supervisor, be sent home for the day and charged with an unexcused absence 1.2.2 Leaving Early (failure to complete a full shift absent legally protected reasons): Half (1/2) point will be issued if leaving early four hours or less and (1) point will be issued if leaving early four or more hours.
