1.2.3 Unexcused Absence : Two (2) points will be charged to the employee for failure to report to work. If the employee fails to provide Proper Notice, one (1) additional point will be charged, absent extenuating circumstances. 1.2.4 No Call/No Show - Unreported Absence: Three (3) points will be assessed to employee’s record, absent extenuating circumstances 1.2.5 Abandonment: An employee who is absent for two (2) consecutive scheduled work days, without notice, shall be considered to have voluntarily terminated his/her employment with the Company. Exceptions to Corrective Action Process steps must be recommended by superintendent and approved by HR Business Partner / Assistant HRBP and/or General Manager. 1.3 DESCRIPTION OF NON-OCCURRENCES Absences Covered by Arizona’s Fair Wages and Healthy Families Act (the PST Act). Hourly employees of the Company are entitled to up to 40 hours job protected, paid sick time (PST) per plan year (Jan 1 – Dec 31) for covered reasons including: the health care needs of the employee or the employee’s covered family member(s), certain facility closures or quarantines during a public health incident, and to tend to the legal, medical, or psychological needs of the employee or an employee’s covered family member (as defined by the PST Act) because of stalking, violence, abuse, or sexual assault. To be eligible to use PST, an employee must have been employed for a minimum of 90 days or must have been hired from a temporary or leasing company and have other benefits waiting periods waived. Only time that is properly taken as Paid Sick Time is protected from disciplinary action. For example, if an employee has a full eight-hour unscheduled absence, but only has available four hours of paid sick leave, the portion of the unscheduled absence not covered by paid sick leave is subject to disciplinary action under the attendance policy. Requirements for taking PST under The PST Act: 1.3.1 You have the right to take up to 40 hours of PST per plan year for yourself or for any family member covered under The Act. 1.3.2 When foreseeable, you must make a good faith effort to provide notice of your need to use PST in advance and you should attempt to schedule the leave in manner that does not unduly disrupt operations. 1.3.3 Requests to use PST may be made orally, in writing, or by any other means acceptable to the Company. While we will not interfere with your right to request use of PST by any means available to you, because of our recordkeeping obligations, we ask that you complete, sign, and return a Paid Sick Time Request Form either when requesting sick time or upon your return to work from PST. Even if you do not choose to complete and/or sign the Paid Sick Time Request form, the Company is allowed to count your absence as PST if we have a legitimate reason to believe that the time is being used in accordance with The PST Act.
